We are seeking a dedicated and compassionate Residential Program Case Manager to join our dynamic team. In this vital role, you will coordinate and oversee individualized care plans for residents within our residential treatment programs, ensuring they receive comprehensive support tailored to their treatment and housing needs. Duties Conduct pre-enrollment screenings including mental health, behavioral patterns, and physical health assessments to inform personalized care plans. Develop and implement individualized treatment plans that incorporate addiction counseling, mental health counseling, and behavioral health strategies aligned with best practices in inpatient settings. Coordinate discharge planning and post program arrangements to ensure seamless transitions for residents moving into permanent housing or community-based services. Collaborate with multidisciplinary teams—including social workers, employment, education, and housing specialist, and other healthcare professionals—to optimize program resident treatment plan. Maintain accurate records, documentation of services provided in compliance with HIPAA regulations. Facilitate intake processes for new residents, ensuring all necessary information is collected efficiently while adhering to records protocols. Participate in diagnostic evaluations and case management activities that support residents' treatment plans within a residential care framework. Requirements Knowledge of addiction counseling techniques combined with social work studies or experience. Experience in program and discharge planning, utilization management, patient service coordination systems desirable. Ability to engage residents in interpersonal examination of barriers to housing. Familiarity with HIPAA regulations ensuring confidentiality and security of persoanl information at all times. Excellent communication skills for collaborating across multidisciplinary teams and engaging with program participants compassionately.
