Full Job Description Job Summary:
Roots Through Recovery is seeking a full-time Registered or Certified MAT (Medication•Assisted Treatment) Counselor/Case Manager with experience working with substance use and co-occurring mental health disorders for our outpatient program in Long Beach, CA. This role performs the core functions of an SUD Counselor/Case Manager with a specialized focus on patients receiving or pursuing MAT services. The MAT Counselor plays a critical role in expediting access to medication support by streamlining intake and engagement processes, providing individual and group counseling tailored to MAT patients, and coordinating closely with medical providers and support staff to ensure continuity of care and medication adherence.
Essential Duties and Responsibilities:
Conduct timely intake, assessment, and orientation for patients referred for MAT services, with a focus on minimizing delays to medication access Coordinate with the Medical Director, prescribing providers, and nursing staff to facilitate rapid scheduling of medical evaluations and induction appointments Provide individual and group counseling to MAT patients addressing substance use, co•occurring mental health conditions, and barriers to medication adherence Develop, review, and update individualized treatment plans and problem lists in collaboration with the patient, reflecting MAT goals and progress Monitor and document patient adherence to prescribed medication regimens, communicating concerns promptly to medical staff Serve as a liaison between medical and counseling teams to ensure coordinated, whole•person care for MAT patients Maintain accurate, timely clinical documentation in the EHR (Sage PCNX) in compliance with SAPC, DMC-ODS, and Joint Commission documentation standards Complete required CalOMS data entry and ensure compliance with admission timelines, including the 7-day admission entry requirement Conduct ongoing risk assessments, crisis intervention, and case management/care coordination, including referrals to ancillary services such as housing, benefits, medical, and vocational support Participate in treatment team meetings, case consultations, and clinical supervision Support patient retention and engagement efforts specific to MAT populations Maintain caseload documentation in compliance with DHCS, SAPC, and TJC regulatory and accreditation standards Perform other duties as assigned
Qualifications:
Registered or Certified SUD Counselor (CCAPP, CADTP, CAADE, or other DHCS•recognized certifying body) in good standing Minimum 1-2 years of experience providing SUD counseling, preferably with MAT (buprenorphine, methadone, naltrexone) populations Working knowledge of MAT modalities, induction processes, and medication adherence support strategies Experience with co-occurring mental health disorders and integrated treatment approaches Strong understanding of
DMC-ODS, SAPC
Provider Manual requirements, and CalOMS documentation standards Excellent interpersonal, organizational, and time-management skills Proficiency with EHR systems (Sage preferred) and Google Workspace Bilingual (English/Spanish) a plus
Physical Demands & Work Environment:
Standard outpatient clinic environment in Long Beach, CA May require occasional travel for field-based services or community partner meetings
