Job Description

Paid Two-Week Training and Onboarding Schedule:
Monday-Friday, 9am-5pm Extensive paid training will be provided, requiring candidates to be fully available for the entire two-week training period, which will be conducted in person. This training will cover essential youth care worker duties, PRO-ACT, Naloxone (NARCAN) training, and crises response to ensure you are well-equipped to deliver high-quality care.
Overview:
Dimondale Adolescent Care Facility is seeking a committed and compassionate Case Manager to join our STRTP team, serving youth ages 12 to 18. This position plays a key role in providing support and structure to adolescents with mental health needs, helping them stabilize and work toward successful transitions.
Caseload:
6 youth (ages 12-18)
Shift:
Mon-Fri, 9am-5:30pm ( Onsite)
Key Responsibilities:
Provide individual and group rehabilitation services, crisis intervention, and intake assessments Coordinate aftercare and community support services Deliver case management services including referrals, advocacy, and linkage to psychiatric, medical, and social services Collaborate with psychiatrists and multidisciplinary teams on treatment planning Maintain clear and timely documentation in compliance with DMH and agency standards Monitor client progress and adapt interventions as needed Conduct comprehensive assessments of youth to identify their needs and strengths. Participate in staff meetings, supervision, and ongoing professional development Maintain a productivity standard of 70% (e.g., 28 billable hours per week.
Requirements:
Bachelor's degree in social work, psychology, or related field One-year direct service experience working with youth with a history of mental illness in a social service or mental health setting A working knowledge of conducting individual and group rehabilitation sessions for those with a history of mental illness and their families. Strong understanding of mental health counseling principles and practices. Must be at least 21 years old DOJ, FBI, and Child Abuse Index clearance required CPR/First Aid certified Valid California driver's license and acceptable driving record to Agency's insurance carrier Ability to work full-time and participate in on-call rotation Experience utilizing Exym is a bonus Must complete Pro-ACT certification within 30 days of hire
Additional Skills:
Experience working with at-risk youth or in a similar social service setting is highly desirable.
