JOB SUMMARY
Works as a member of an interdisciplinary team to develop a discharge plan and increase consumer's knowledge of community services, how to access such services and the importance of these services. This position involves participation and implementation of program goals consistent with preventative and holistic care, having objectives to promote a strengths-based, wellness-informed consumer- centered approach to assisting individuals in achieving person-driven and self-management goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include, but are not limited to, the following: Provides community linkage services to the consumers and assists with transportation with the consumers when related to linkage activities. Provides individual and/or group training in community resource linkage and development skills. The incumbent may facilitate one or more therapeutic groups per week when necessary. Participates in regular clinical supervision attends clinical case conferences and staff meetings to gain knowledge and communicate program and individual needs. Completes a transition plan with the client that focuses on development of a discharge plan to a lower level of care. 6. Develops, implements, and administers therapeutic recovery plan by performing intake assessments (BioPsychoSocial Assessment) for psychological history and compiles all pertinent documents to facilitate treatment plan. Assist the treatment team in the preparation of the initial recovery plan and the master treatment plan, to include scheduling of treatment team meetings, working with the parents, families, and others to encourage active involvement in treatment of the consumer. Assists in crisis situations as appropriate, following emergency protocols and procedures. Schedule and coordinate off grounds medical and clinical appointments, verify insurance coverage or payment arrangements, coordinate appointments with the transport team, and prepare copies of any forms and instructions necessary to take with assigned staff to the appointment. Respects/honors individuals rights and responsibilities & demonstrates professional boundaries & ethics. Documents and maintains clinical records in a manner that assures compliance with all agency policies and procedures and local, state, and federal regulations. The incumbent may represent the program at community, state, and /or national. Promotes a recovery, wellness and healthy lifestyle, including drug free.
QUALIFICATIONS

Must have lived experience:
having personally lived with mental health or addiction challenges and/or a consumer of mental health/chemical dependency services OR a family member/peer of a mental health or substance abuse consumer. If lived experience, must be able to model recovery experience for more than one year. Must be at least 18 years of age; required health screening, TB clearance, criminal record clearance. 1.
EDUCATION/EXPERIENCE
Education/Expierence in psychology, counseling, social work, or related field required. . If an employee does not have a minimum of one (1) year fulltime experience working in a program serving people with a psychiatric diagnosis, then a plan of supervision is required and will be put into place. 2.
LICENSES/CERTIFICATIONS
Valid State Driver License Valid First Aid / CPR certified. 3.
LANGUAGE/CULTURAL SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and policy/procedure manuals. Ability to write routine reports, correspondence, and document consumer behavior. Ability to speak effectively before groups of consumers or employees of organization. Bilingual preferred. Solid understanding of a variety of cultures and able to effectively communicate, understand and correspond with those cultures.
