Endura Healthcare is seeking a dedicated and compassionate full-time Social Services Resource to join our dynamic team. $85,000-$100,000 annually. Pay is dependent on applicant's relevant experience. Responsibilities Conduct thorough patient assessments to identify social, emotional, and environmental needs, utilizing clinical research methods and behavioral health expertise. Coordinate with healthcare providers to develop personalized care plans that incorporate social work principles, mental health counseling, and crisis intervention strategies. Facilitate access to community resources such as hospice care, assisted living, special education, and home & community care services. Maintain accurate medical documentation and records in compliance with HIPAA regulations, utilizing EMR systems for data collection and case management. Provide individual and group counseling sessions employing techniques like motivational interviewing, cognitive behavioral therapy, and behavioral therapy to support mental health and addiction recovery. Assist with intake procedures and diagnostic evaluations for patients with disabilities, autism spectrum disorder, dementia, PTSD, or developmental disabilities. Collaborate with interdisciplinary teams to ensure seamless patient care across inpatient, outpatient, nursing home, and post-acute settings while managing crisis situations effectively. Qualifications Proven experience in social work or clinical counseling within hospital settings or community-based programs. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management. Expertise in child & family counseling, behavioral health interventions, and working with individuals with disabilities or developmental challenges. Familiarity with EMR systems and HIPAA compliance standards for secure data handling. Excellent communication skills for intake assessments, group therapy facilitation, and patient education. Ability to perform diagnostic evaluations and case management tasks with sensitivity and professionalism. Prior experience in hospice care, geriatrics, pediatrics or Alzheimer's/dementia care is highly desirable. Join us in delivering compassionate support that transforms lives!
