In this role, you will provide outpatient psychotherapy, clinical assessments, treatment planning, and ongoing support to clients across diverse populations, including children, adolescents, adults, families, and older adults. This position is ideal for a clinician who values trauma-informed, culturally responsive, low-barrier care and wants to be part of a collaborative behavioral health team committed to accessible mental wellness services in the Pueblo community. About Us Elevate Mental Wellness is a growing outpatient behavioral health practice serving communities across Colorado, including Englewood, Colorado Springs, and Pueblo. We provide accessible, compassionate, and culturally responsive care to individuals and families with a wide range of mental health and psychosocial needs. Key Responsibilities Provide individual, family, and group therapy in an outpatient behavioral health setting Conduct intake assessments, biopsychosocial assessments, diagnostic evaluations, and ongoing clinical assessments Develop, update, and implement individualized treatment plans Provide trauma-informed, culturally responsive, and client-centered care Use evidence-based or clinically appropriate interventions, such as CBT, DBT-informed skills, motivational interviewing, trauma-informed therapy, supportive psychotherapy, and other appropriate modalities Support clients with concerns related to anxiety, depression, trauma, grief, family stress, behavioral concerns, life transitions, substance use concerns, and social determinants of health Provide crisis intervention, safety planning, referrals, and coordination of higher levels of care when clinically indicated Maintain accurate, timely, and HIPAA-compliant documentation in the EMR Use appropriate ICD-10 diagnostic coding and maintain compliance with Medicaid, commercial insurance, and regulatory documentation standards Coordinate care with psychiatric providers, case managers, families, schools, hospitals, DHS, probation, and community partners as appropriate Participate in team meetings, case consultation, care coordination, and quality improvement efforts Provide telehealth services when appropriate and approved based on client needs and clinic requirements Contribute to a supportive, ethical, and team-based clinical environment Qualifications Active Colorado license as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) Experience working with children, adolescents, adults, families, trauma, anxiety, depression, and complex psychosocial needs preferred Strong clinical judgment, assessment, documentation, and treatment planning skills Familiarity with ICD-10 coding, CPT documentation requirements, Medicaid standards, HIPAA, and electronic health records preferred Ability to work independently while collaborating with a multidisciplinary team Commitment to trauma-informed, low-barrier, culturally responsive, and client-centered care Telehealth experience preferred Schedule & Work Environment Full-time preferred Outpatient setting In-person at the Pueblo Fortino location Hybrid flexibility may be considered based on clinic needs and performance Supportive administrative and clinical team environment How to Apply Submit your resume and a brief introduction.
