In this vital role, you will provide comprehensive social support, conduct assessments, and coordinate care plans to promote well-being across diverse populations. Duties Conduct thorough patient assessments, including intake interviews and diagnostic evaluations, to determine individual needs and develop tailored care plans. Provide clinical counseling, behavioral therapy, and mental health counseling to individuals and groups, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and applied behavior analysis (ABA). Coordinate with multidisciplinary teams to ensure seamless case management, patient care, and documentation in compliance with HIPAA regulations. Assist with crisis intervention efforts, including de-escalation techniques and emergency response procedures for patients experiencing mental health crises or behavioral incidents. Collect and maintain accurate medical records, including progress notes, treatment plans, and medical documentation using electronic medical record (EMR) systems. Support patients with disabilities or developmental delays through specialized services such as autism spectrum disorder interventions or dementia care strategies. Engage in community outreach and home-based services to deliver hospice care, post-acute care, or senior care in various settings including assisted living facilities and inpatient units. Experience Prior experience working in social work, behavioral health, or mental health counseling environments is highly desirable. Demonstrated knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical records management. Experience with crisis management, trauma-informed care (including PTSD care), and working with individuals with disabilities or developmental conditions such as autism or Alzheimer's disease. Familiarity with hospital settings, outpatient clinics, nursing homes, or home & community care environments enhances your ability to deliver effective support. Proven ability to handle sensitive situations with professionalism while maintaining confidentiality in accordance with HIPAA standards. Strong skills in data collection for clinical research or quality improvement initiatives are a plus.
