Salary:
$48,300 $3,000 sign on bonus! About the Program This role supports the IICAPS model, providing intensive home?based services to children at risk of psychiatric hospitalization. Services include individual and family therapy, parenting skills training, 24/7 crisis support, case management, and advocacy. You will work as part of a two?person team (Clinician + Mental Health Counselor) and collaborate closely with DCF, schools, community providers, and psychiatric partners. Staff participate in weekly Rounds, group supervision, and maintain documentation aligned with licensing, Medicaid, accreditation, and Yale model requirements. The goal: stabilize the child in the home, strengthen family functioning, and support transition to traditional outpatient care.
Position:
Family Support Specialist Job Summary The Family Support Specialist provides case management, support services, and mental health services to children and families in office, home, and community settings. This role ensures coordinated care across agency and community partners while meeting all documentation and program requirements. Key Responsibilities Comprehensive assessments and family engagement Treatment planning and service contracting Implementing and adjusting treatment plans as needed Discharge planning and linkage to community services Case management and advocacy Community collaboration with schools, courts, hospitals, and partner agencies Accurate, timely clinical documentation Active participation in team meetings, supervision, and agency initiatives Community representation Experience and Skills Required Education BA in Human Services or BSW required Experience & Competencies 3 years' experience in human services preferred Strong oral and written communication skills Ability to work independently and manage multiple priorities Cultural competency and experience with inner?city youth Bilingual (Spanish) preferred Commitment to Performance?
