Full Job Description Position Summary :
Behavioral Health Providers, Clinicians, and/or Licensed Professional Counselors / LAPC / LMSW / LCSW / LMFTs / LCSWs most often work in mental or behavioral health counseling centers and/or group practices. Licensed clinicians often interview patients, give diagnostic tests and assessments, provide individual, family, or group psychotherapy, design and implement behavior modification programs, and may make determinations about potentials of high risk or treatments of hospitalization. Some clinicians collaborate with physicians and other specialists to develop and implement treatment and intervention programs that clients can understand, agree, and comply with. Assure programmatic requirements are met based on various oversights/funding entities:
DBHDD, CMS, HUD, DCA.
Provide budget oversight & monitoring and also development & monitoring of programmatic & financial outcomes reports. Assure that staff is appropriately trained and current in training. Provide Human Resources management skills (in conjunction with the HR Department).
Key Duties and Responsibilities System of Care Coordination:
Enhance the System of Care framework and case management for at-risk youth, emerging adults, and families. Expand school and community collaboration within the region serviced, by: establishing a greater presence in local interagency planning meetings and other community support meetings, identifying and nurturing key relationships with staff in current school systems, develop and operationalize local social marketing, collaborate with community partners for educational and employment supports, and oversee case management team and series for at-risk you and emerging adults. Provide training, technical assistance and workforce development opportunities to local system of care partners, implement a community wide training curriculum that would incorporate training on evidence based programs for partner organizations. The System of Care Coordinator will interview clients and give diagnostic tests and will provide outpatient and/or in-home services, including individual and family therapy, group therapy and may perform psychosocial assessments, crisis assessments, ANSA, CANS, and treatment plans. The coordinator will also be responsible for updating individual service and treatment plans, communicating with carious referral agencies including DFACS, Juvenile Courts, and Schools, and working with a team of Community Support Individuals and collaborating with other mental health professionals. Assist with personal development and work performance of the client, and other behavioral, therapeutic interventions and support. Support and intervene in crisis situations. Provide documentation notes that relate to the customer-driven mental health treatment plan.
Minimum Qualifications:
Masters Degree from an accredited institution in psychology, social work, counseling or marriage or family therapy. License eligible in the state of Georgia.
