Job Description

JC Lewis Primary Health Care Center is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay. The J.C. Lewis Primary Health Care Center excels in the provision of compassionate care and promotes healing through healthy living. Our team of highly skilled providers partners with patients in need to enable them to achieve and maintain optimal health and wellness across their life cycle. JC Lewis Primary Health Care Center is seeking a skilled Licensed Professional Counselor to join our healthcare team. The Licensed Professional Counselor (LPC) functions as an active member of the integrated care clinical team by assuming the following non-inclusive list of primary responsibilities
Primary Responsibilities:
Conduct comprehensive assessments for mental health and/or substance abuse treatment to consumers of the Health Center, to include adults, children, adolescents and families. Provide individual and family counseling, crisis intervention, and facilitation of psycho-educational groups and support groups Engage in direct consultation with primary care providers, nurses, psychiatrists and other primary and behavioral health care staff to ensure continuityintegration of services Maintain knowledge of and ensure compliance with all grant requirements Provide clinical oversight to other behavioral health providers on the integrated care team, to include coaching, mentoring and guidance of newer clinicians and interns, as needed. Promote education and awareness concerning signs and symptoms of behavioral health issues among the child and adolescent population to the medical staff and general community Develop relationships with representatives in other services/agencies to support individuals in obtaining additional services such as housing, additional mental health care, psychiatric medication, specialty medical care, financial assistance, legal advocacy, etc.
Qualifications:
Current license in the State of Georgia (LPC) Master's degree in counseling, Social Work or related field from an accredited college or university At least 2 years' experience providing mental health and substance abuse services to children and adolescents Effective verbal and written communication skills Ability to build rapport, trust and comfort with patients Strong team/consensus building skills Ability to effectively resolve conflict and cope with crisis situations Excellent assessment, time management, documentation and organization skills Must pass a National Criminal Background Check and Drug Screening Other Duties and Responsibilities Performs clerical duties and completes reports as assigned Maintain certification/licensure, to include the procurement of appropriate CMEs Participate in agency meetings, in-service trainings and workshops as deemed appropriate All other duties as assigned
