Job Description
Residential Locations:
Maui #CareHawaiiLP Position Summary Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Counselor to support consumers with addiction and mental health. The Counselor is a member of a multidisciplinary team, responsible to carry out rehabilitation and support functions such as psychosocial rehabilitation programming, treatment planning, education, support, consultation with families, and providing crisis intervention under the direction of the clinical staff. Additional duties include collaboration with case managers and external treatment teams, processing utilizations, admission and discharge paperwork and preparing reports. Position QualificationsMinimum Education:
Must have a High School diploma or equivalent.Minimum Field-of-Experience:
One (1) year experience preferred but not mandatory in the provision of psychological, social, or counseling services in a human service delivery setting that includes interviewing consumers and/or observing and recording behaviors for use in the development and implementation of treatment, rehabilitation, or service plans.Preferred Experience:
One (1) year work experience with adults with serious mental illness, co-morbid medical conditions, including physical disabilities. A plus if CNA certified.Required Certification/Registration:
○ CPR ○ First Aid ○ CPI ○ TB ○ Physical Exam Job Competencies Required Skills- Mature, dependable, and responsible.
- Must be comfortable working with adults with a history of severe mental illness and their families.
- Must be culturally sensitive and aware.
- Must know the basics of ethics, HIPPA standards, and problem solving.
- Must display sound judgment, a decent work ethic, and personal integrity.
- A working knowledge of diverse populations including treatment of behavior disorders, mood disorder, anxiety disorders and Schizophrenia/Schizoaffective disorder. Technical Skills
- Basic use of Microsoft Word Suite, fax machine, telephone, DVD player, and television. Working Environment & Demands Environment
- Able to work indoors and/or outdoors in a well vented or airconditioned area, possibility of visiting and/or working at other residential sites.
- Exposure to infectious disease and/or viruses is a risk.
- The noise level in the environment is usually moderate. Special Demands
- Ability to respond effectively to the varied needs and demands of consumers, families/significant others, staff, physicians, and other multidisciplinary team members.
- Must have a strong commitment to the right and ability of each person with a severe mental illness to live in normal community residence, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services. Consumers Served
- Employee will service adults diagnosed with a serious mental illness (SMI), who are at least 18 years of age.
- Consequently, the employee must be competent in the interpretation of client's self-report or behavior.
- The treatment of clients should reflect an understanding of the developmental needs and range of treatment needs for each client.
- Be a member of the treatment team and participate in treatment planning for clients.
- Participate in developing, writing, implementing, evaluating, and revising overall treatment and / or housing goals and plans in conjunction with the treatment team.
- Participate in comprehensive assessment of psychiatric history, mental status, and diagnosis; physical/dental health; use of drugs or alcohol; education/employment; social development and functioning; activities of daily living; and family structure and relationships.
- Function as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Document client progress towards treatment goals and maintain a permanent record of client's daily activities according to established methods and procedures.
- Participate as scheduled in staff organizational meetings and treatment plan review meetings.
- Participate in facilitating various psychosocial rehabilitation programming/treatment groups for clients.
- Provide ongoing assessment and response to treatment.
- Perform job coaching, problem solving, and support on and off the job site.
- Aid with nutritional education/food preparation, housing placement, money management skills, transportation options.
- Provide support, advocacy, and coordination of social, vocational and activities of daily living needs.
- Ensure participation in the completion of shift change duties.
- Responsibly reports to work on time/consistent attendance and follows program protocol when calling in sick and or for emergencies.
- Communicates effectively, and respectfully with all staff, clients, and POS providers.
- Maintains a positive and enthusiastic attitude at work.
- May perform special tasks of nursing care as delegated by the RN as allowed under the subchapter 15 Section 16-89, Hawaii Administrative Rules.
- Ensure cleanliness and organization of housing site to include actual cleaning when deemed necessary.
- Assist with managing crisis situations at housing sites.
- Participates in health and safety requirements at housing sites and consistently reports any health and safety deficiencies.
- Regularly conduct and/or ensure performance of all health and safety activities inclusive of but not limited to all health and safety drills.
- Participate in AMHD funded training and in-service training as assigned and delegated by supervisors.
- Maintains a current Physical Evaluation, TB Clearance, CPI, First aid and CPR certification.
Pay:
