Bilingual Family Support Worker - West Side Clinic Primary Health Care Inc - 3.0 Clive, IA Job Details Full-time $15.50 - $23.25 an hour 7 days ago Benefits Health savings account Paid holidays Disability insurance Health insurance Dental insurance Tuition reimbursement Paid time off Vision insurance 401(k) matching Pet insurance Qualifications Spanish English High school diploma or GED Full Job Description As a Family Support Worker, you will assist patients by identifying needs, matching needs with community resources, and applying for entitlement programs, while continuously ensuring a high level of customer service. You will serve as an advocate for a culturally diverse patient population and refer patients to external community agencies as needed. You will coordinate with team members to provide positive service experiences for patient and will serve as a resources for the team about community resources. Verbal fluency in both Spanish and English is required for this position. What's Great About This Position Enjoy work-life balance with a stable work schedule. This position typically works 40 hours per week, Monday - Friday 8 AM - 5 PM. Earn a generous PTO accrual totaling 4 weeks off by the end of your first year of employment. What You Will Do Assists uninsured individuals and families in identifying needs and provides access to mainstream resources, medical insurance and other entitlements, including but not limited to BCCEDP and VPN. Assists clients in completing applications for different entitlement programs. Conducts outreach activities to uninsured individuals through community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues. May provide assistance at PHC clinics, at community agencies and in client homes. Responds to requests for assistance from, and facilitates referrals to PHC clinics, community agencies, and other resources. Collaborates with members of Patient Care Teams, supports workflows that ensure maximum staff and patient satisfaction and organizational financial success. Documents all client contacts in compliance with PHC policies, also including data entry as needed. Participates in staff and performance improvement meetings and trainings as requested. Provides supportive services in connection with clinic operations as requested, which may include interpreting or translating and supporting the front desk operations as needed. Performs other duties as assigned. Qualifications You Need to Bring Required High school diploma or equivalent. Minimum 1 year experience in human services, healthcare, or related field. Bilingual, English and Spanish verbal and written language proficiency. Knowledge of community resources, including entitlement programs. Effective verbal and written communication skills. Professional skills with ability to maintain composure in all situations and interact empathetically. Strong interpersonal skills with ability to establish and maintain effective working relationships. Reliable transportation to perform outreach activities at off-site locations. Must be obtained within 90 days of employment: Current Mandatory Reporter training approved by the State of Iowa Preferred Experience working with underserved population. Experience in a community health center or public health setting. We Take Care of Our People Your experience and skills determine your base pay. The hiring range for this position is typically $15.50 - 19.38, though highly experienced candidates may earn up to $23.25 per hour.
