JOB DESCRIPTION
SUMMARY
OF POSITION
Provides therapeutic interventions aimed at fostering personal growth and resilience in clients with behavioral health diagnoses. Conducts ongoing assessments of client needs, maintaining accurate documentation, and developing personalized growth plans tailored to therapeutic requirements. This role involves significant decision-making regarding the implementation of interventions and adjustments to treatment plans based on client progress.
ESSENTIAL DUTIES & RESPONSIBILITIES
Engages with newly assigned clients and their families by documenting contact within 2 business days of the referral. Conducts intake for clients entering services, ensuring understanding of all forms and obtaining necessary signatures. Develops individualized care plans that are person centered, family centered, and service centered. Assists clients in linking with internal and external resources as outlined in service plans. Maintains documentation of services provided and submits for review as required. Monitors client progress in treatment and participates in service plan reviews. Provides community support, care coordination/case management, and crisis intervention services.
KNOWLEDGE, SKILLS & ABILITIES
Knowledge of community geographical awareness and of community and agency resources. Skill in interviewing, assessment, and counseling. Proficiency in customer service techniques, demonstrated through effective resolution of client issues. Advanced problem-solving skills, evidenced by successful project outcomes. Familiarity with digital communication systems, including Electronic Health Record for managing client interactions. Ability to conduct thorough assessments and form diagnoses, utilizing standardized tools and methodologies. Expertise in virtual communication platforms, ensuring clear and effective remote interactions. Strong analytical skills, applied in evaluating individual needs and resources.
