Job Description

We are seeking several compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide comprehensive mental and substance use health services within a multidisciplinary healthcare setting for all of CHP sites. The ideal candidate will deliver evidence-based psychotherapy, conduct clinical assessments, and develop individualized treatment plans to support patients across diverse populations, including children, adults, and geriatrics. This role offers an opportunity to make a meaningful impact through patient-centered care, crisis intervention, and case management, utilizing advanced clinical skills in behavioral health and social work. Duties Conduct thorough patient assessments, including diagnostic evaluations using ICD-10 and ICD coding systems, to inform treatment planning. Provide individual, group, and family therapy sessions utilizing modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, applied behavior analysis (ABA), and crisis intervention techniques. Manage cases involving patients with developmental disabilities, autism spectrum disorder, PTSD, Alzheimer's disease, and other chronic or acute mental health conditions. Collaborate with healthcare teams to coordinate patient care plans, including medication management in conjunction with psychopharmacology providers. Maintain accurate medical documentation adhering to HIPAA regulations and utilize Electronic Medical Records (EMR) systems for data collection and record keeping. Engage in clinical research activities related to behavioral health and mental health counseling to support continuous quality improvement. Provide crisis management services and respond effectively to urgent mental health needs. Offer specialized counseling such as Christian counseling or addiction counseling as appropriate. Participate in case management activities to connect patients with community resources and support services. Ensure compliance with all regulatory standards including CPT coding and medical terminology documentation. Qualifications Valid Licensed Clinical Social Worker (LCSW) license in the state of practice. Proven experience in psychotherapy, mental health counseling, or behavioral health settings. Strong knowledge of diagnostic evaluation tools including
ICD-9/ICD-10
coding systems. Experience working with children, adolescents, adults, or geriatrics; familiarity with pediatrics and special education is a plus. Competency in crisis intervention, trauma-informed care, PTSD care, addiction counseling, and working with individuals with disabilities or autism spectrum disorder. Proficiency in EMR systems and medical documentation practices ensuring HIPAA compliance. Excellent communication skills for intake assessments, patient education, and interdisciplinary collaboration. Ability to handle sensitive situations with professionalism and empathy while maintaining confidentiality. This position is integral to providing holistic mental health services that improve patient outcomes through compassionate care and clinical excellence.
