Full-time, Monday-Friday, 8:00 AM-4:30 PM Join our team at the Robert Young Center as a Mental Health Professional! The Mental Health Professional (MHP) provides community-based case management services to adults with severe mental illness using Trauma-Informed Care principles. Under the supervision of a Licensed or Qualified Mental Health Professional, the MHP completes assessments, assists with treatment planning, monitors client progress, and provides crisis intervention as needed. The MHP collaborates with multidisciplinary teams to coordinate care and support client recovery. Responsibilities also include maintaining accurate clinical documentation, coordinating services, and demonstrating Trinity's values in all interactions with clients, families, and staff. Provides case management services, including assessment, treatment planning, and ongoing support for clients with behavioral health needs Conducts diagnostic assessments and develops individualized treatment plans Coordinates care and collaborates with multidisciplinary teams, including providers, social workers, and nursing staff Delivers counseling, crisis intervention, and community-based support to clients and families Monitors client progress and adjusts treatment plans based on clinical findings Maintains accurate, timely clinical documentation in compliance with regulatory standards Qualifications:
Education:
Requires a high school diploma or GED and a minimum of five years documented clinical experience in mental health or human services; OR An individual possessing a certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in provided mental health services; OR An individual possessing a bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or a related human service field. License(s)/Certification(s): Valid driver's license. (CPI) training within 6 months of hire. Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS) Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs.
