Job Description

Position Summary:
As part of the integrated health model of Heart City Health Center, the position provides integrated behavioral health services to patients currently receiving primary care services at HCHC. The Licensed Mental Health Therapist works as an interface for patients accessing services such as individual, peer/family counseling and support, screening, assessment, brief treatment, consultative services, and education. This position works directly with primary care providers to strengthen relationships between behavioral health and primary care services at Heart City Health Center.
Essential Functions:
Comply with established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and customer service standards. Provide ongoing consultation to primary care services for appropriate mental health interventions.

Participate in ongoing, current behavioral health education, training, and skills. Provide training to primary care clinic staff in behavioral health topics including, but not limited to, counseling skills and confidentiality, substance use and misuse, including medication, wellness, and self-care, self-sufficiency, rehabilitation, and recovery. Ability to conduct brief, non-stigmatizing screenings to evaluate the needs of adults, adolescents, and children for behavioral health and support services. The Evaluations based on medical record, staff, direct interview, collateral information and other reliable secured sources legally allowed. Ability to perform comprehensive, bio-psychosocial assessment for mental health, substance use disorder, co-occurring disorders, domestic violence and medical needs. Conduct follow-ups based on results of evaluation as needed. Provide early interventions, referrals to specialty mental health services, referral to more intensive psychiatric/psychological as needed.

Electronic medical record system (EMR) documentation according to established protocol, state/and national regulations, document findings in patients medical record according to established protocol. Participation in monthly Provider meetings. Provide consultation and case by case consultation on behavioral/mental health issues affecting patients, as needed. Provide training to primary care staff on diagnosis and treatment, medication management, confidentiality, and other behavioral health topics as needed. Participate with other staff in multidisciplinary staff meetings, retreats and other staff processes to achieve program development, staff development, program evaluation, program operations review, and other activities needed to maintain quality of early intervention services. Maintain confidentiality regarding privileged administrative and client information in a professional manner. Other duties as assigned.

Education, Experience and Licensure:
A Licensed Mental Health therapist (LMHT), LMHC, LMFA, (LCSW) required.

Experience (2 years or more preferred) serving clients with behavioral disorders, psychological illness, and /or substance use disorders. Strong clinical skills with experience in outpatient setting required.

Demonstrated knowledge about behavioral disorders, psychological illness, and substance use disorders including, motivational interview engagement, de-escalating strategies, and knowledge and skill in the use of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Must have strong commitment to client rights, confidentiality, cultural Humility and ethical Standards. Experience working within an integrated behavioral Health (IBH) and primary Care setting preferred, including collaboration with Medical Providers and interdisciplinary teams.
