Job Description

Essential Functions Provides for the safety, stability and well-being of children and families during the referral period Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within in 20 days of referral; case closure within 3- 6 months of referral Engages and works in partnership with case manager and families to complete assessments Meets with family in the home weekly to monitor family progress towards their case plan goals and assesses each child's safety Assesses each child's safety with every family contact and reports all suspected child abuse / neglect Provides support and resources to family in Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication Works closely with the Case Manager to provide services for the family and make decisions about child well-being Remains current on the availability of community resources and assists all families in obtaining assistance Acts as a liaison with schools to obtain child records, attend conferences, track attendance and support achievement Acts as a liaison with Behavioral Health and Child Placement Agency to support treatment and respite plans Provides transportation as necessary to meet established program achievement and fidelity goals Collects drug screens as necessary Documents case activities, information weekly Appears and testifies in court as required Responds promptly to family crisis situations, documents incidents and is available 24/7 to clients Promotes and practices teamwork in all activities Fosters and maintains professional relationships with families, community partners and peers Participates in professional development that is relevant Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required Please note this description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job.
