Overview We are seeking a licensed and compassionate Substance Use Counselor to join our team at a 10-bed Addiction Detox Center . The ideal candidate will provide clinical counseling and support to individuals in the early stages of recovery, many of whom present with co-occurring mental health conditions. This role places a strong emphasis on intake assessments, comprehensive treatment planning, coordinated case management, and aftercare planning to ensure continuity of care beyond detox. The Substance Use Counselor will conduct biopsychosocial assessments, collaborate with the multidisciplinary team, and deliver evidence-based therapeutic interventions within a structured detox setting. This position plays a critical role in stabilizing patients, supporting safe transitions to the next level of care, and promoting long-term recovery outcomes. •Must possess a Bachelors or Master's degree in Social Work, Counseling, or a relevant behavioral healthcare field•Responsibilities Conduct daily individual meetings with patients to assess clinical progress, address concerns, and provide therapeutic support throughout the detox process. Perform comprehensive intake and ongoing assessments , including diagnostic evaluations using ICD-9 and ICD-10 coding standards. Develop, implement, and regularly update individualized treatment plans utilizing evidence-based modalities such as CBT, motivational interviewing, behavioral therapy , and other clinically appropriate interventions. Provide individual therapy and facilitate group and family counseling sessions focused on addiction recovery, stabilization, mental health support, and crisis intervention. Deliver effective crisis management and intervention during acute behavioral health or substance-related episodes. Lead and coordinate case management services , including identifying and resolving psychosocial, clinical, and logistical barriers to continuing treatment. Actively encourage and motivate patients to engage in aftercare programs , including residential treatment, outpatient services, sober living, and community-based recovery supports. Collaborate closely with the multidisciplinary treatment team to coordinate aftercare planning, discharge planning, and continuity of care . Assist patients in eliminating barriers to treatment continuation , such as transportation, insurance challenges, housing instability, and access to follow-up care. Maintain accurate, timely, and compliant clinical documentation in the medical record in accordance with HIPAA regulations , medical terminology standards, and CPT coding requirements. Coordinate with hospitals, community providers, social services, and referral partners to support seamless transitions to the next level of care. Utilize clinical data, research, and ongoing assessments to monitor treatment outcomes and adjust interventions as needed. Participate in ongoing professional development to remain current in addiction medicine, behavioral health, trauma-informed care, and PTSD treatment.
