Positions available in Lafayette Office.
Serving the following parishes:
Lafayette, Iberia, St. Martin, St. Landry, Acadia. Responsibilities and Duties Provide direct member services, individually or in small group settings. Provide skills development according to Individual Service Plan (ISP) under the direction of the LMHP. Monitor member's progress toward established goals in the ISP . Assist in coordinating services for the member and family within the community, following the strategies developed in the ISP . Assists LMHP in compiling updated information for ISP reassessment of the member's progress toward goals. Maintain a Daily Log to document type and time of services provided to members throughout the workday. Document services provided in Carelogic. Maintain calendar of services to be provided. Perform all other duties as assigned by immediate supervisor, Clinical / Compliance Director and/or CEO.
Qualifications and Skills Education:
Bachelor or Master of Arts or Science degree in social work, counseling, psychology or sociology, rehabilitation services, health sciences, recreational therapy, special education, early childhood education, secondary education, family and consumer sciences, criminal justice, or human growth and development. English language reading, writing, and speaking skills Excellent communication and listening skills Neat and legible writing Computer literate Knowledge and experience working with general office equipment (phones, copier & fax) Good organizational and prioritization skills Detail oriented with the ability to follow through a given task Must maintain appropriate level of professional demeanor Must maintain appropriate composure when dealing with sensitive subject matter Must recognize, retain and preserve confidential information

Pass criminal background check Pass drug screening Be eligible to provide Medicaid services Benefits Full time employees qualify for retirement plan after one (1) year; quarterly bonuses and annual raises. All employees qualify for mileage, which is paid monthly. Company also pays up to $20 monthly towards health club or weight loss program. Company provides a tablet for use that includes unlimited texting, talk and data. Full time employees also receive paid holidays and accrue sick leave and vacation provided you work 80 hours per pay period. Pay period is every two weeks. A week for us is from Monday through Sunday. Overtime available at time and a half hourly rate.
