CLINICIAN
Substance use treatment and integrated dual-diagnosis support Program of Assertive Community Treatment (PACT) (Mobile position in the communities surrounding Bedford, MA)What You Will Do:
As a PACT Clinician, you will be part of a multidisciplinary team providing recovery-oriented, community-based mental health services to adults living with serious mental illness. You will build and maintain therapeutic, supportive relationships with persons served, and work collaboratively to promote independence and recovery in all areas of life.Key responsibilities include:
Supporting individuals in identifying recovery goals and developing person-centered, individualized action plans (IAPs) Delivering clinical services in homes and community settings, including assistance with appointments, activities, and transportation Providing psychoeducation, support, and coaching in areas such as money management, self-advocacy, and symptom management Collaborating with team members to provide integrated care and coordinate services Follow the Evidenced-Based Assertive Community Treatment model to support individuals in Psych-Rehab.Who You Are:
Master's degree (If clinical degree, rate increases withLCSW, LICSW, LMHC
) At least one year of experience in the behavioral health field Experience with public benefits and entitlements Strong communication, documentation, and organizational skills Skilled in Microsoft Word and other basic technology tools Valid driver's license and reliable transportation required Passionate about recovery-oriented care and working with individuals in a community settingWho We Are:
