In this role, you will provide comprehensive mental health and behavioral support directly within clients' homes, fostering a safe and nurturing environment for individuals of all ages. Your expertise will help clients develop coping skills, manage behaviors, and improve overall well-being through evidence-based interventions. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse needs, including those with developmental disabilities, autism spectrum disorder, or mental health challenges. The ideal candidate is energetic, motivated, and passionate about delivering personalized care that promotes independence and resilience. Responsibilities Conduct thorough patient assessments to identify needs and develop tailored treatment plans using diagnostic evaluation techniques. Deliver individual and group counseling sessions utilizing therapeutic modalities such as cognitive behavioral therapy (CBT), behavioral therapy, and applied behavior analysis. Implement evidence-based interventions including motivational interviewing, art therapy, and crisis management strategies to support clients' mental health and behavioral goals. Manage case documentation accurately by maintaining HIPAA-compliant medical records, progress notes, and treatment plans. Collaborate with families, caregivers, educators, and other professionals to coordinate home and community-based care that aligns with each client's unique circumstances. Provide crisis intervention when necessary to ensure immediate safety and stability for clients experiencing mental health or behavioral crises. Collect data on client progress to inform ongoing treatment adjustments and ensure measurable outcomes are achieved. Utilize conflict management skills to navigate challenging situations effectively while maintaining a positive therapeutic environment. Engage in ongoing professional development related to working with individuals with disabilities, autism spectrum disorder, PTSD care, and other specialized areas of behavioral health. Skills Proficiency in motivational interviewing techniques to foster client engagement and motivation for change. Extensive experience working with individuals with developmental disabilities, autism spectrum disorder, or other special needs populations. Strong background in addiction counseling a plus as well as and crisis management strategies. Knowledge of social work principles combined with expertise in child & family counseling and behavior management. Ability to perform patient care tasks including patient assessment, psychotherapy, and diagnostic evaluation within a home setting. Familiarity with medical terminology, medical documentation standards, and HIPAA regulations to ensure confidentiality and compliance. Skilled in conflict resolution, crisis intervention, and de-escalation techniques to manage challenging behaviors effectively. Experience applying behavioral health interventions such as applied behavior analysis, cognitive behavioral therapy (CBT), and trauma-informed care like PTSD support. Capable of working independently in home environments while maintaining high standards of professionalism and client-centered care. Strong organizational skills for data collection, progress tracking, and case management responsibilities.
