Non-Exempt Job Summary:
The Family Support Coordinator is a liaison between the Department of Developmental Services and the families we serve. In collaboration with families served, will help develop a family support and expense plan as well as complete assessments to determine the needs of a family, both in the home and the community. They will lead the implementation of the family support plan as well as assist the family in applying for MassHealth, Social Security, SNAP, and other public benefits. They will also attend meetings and advocate on behalf of the family. Often the "first face" a new family sees; the coordinator will encourage families in their efforts to promote the inclusion of their family members with disabilities in education, recreation, and all other aspects of community life. The coordinator responds to family-identified needs and direct input from individuals with disabilities, offering families and individuals the opportunity to exercise control and direction over the support available to them.
Essential Job Functions:
Intake of newly eligible families to the Family Support Center Collaborate with families to complete Family Support Plans and annual DDS documentation Collaborate overall support with clients and their parents, and/or caregivers, DDS Service Coordinators, and other support professionals Provide a range of services, including information, referral, activities, along with providing support to families to enable individuals with disabilities to remain in their own homes. Timely and accurate documentation and record keeping Complete and track the Flexible Funding Expenditure Plan with families and record uses of funding utilizing agency provided Excel spreadsheets Ability to be mobile and to meet families in their homes and local communities Evening or Weekends hours are based on Social Recreation or company-wide events sponsored by the agency
Qualifications:
Education & Experience Required:
Bachelor's Degree or a minimum of three years' experience working in the human service field. Understanding of supports and services needed by families in caring for disabled members. Excel-ability to accurately enter data Maintain CPR/First Aid certifications, Safety-Care training, agency-required trainings Excellent listening and communication skills Proficient using Microsoft Office and able to learn other office computer software Must have a current, valid state-issued driver's license.

Work Location:
Family Support Center, currently located in Weymouth and surrounding areas to meet families in their homes. Must be able to lift up to 25 pounds Must be able to assume and maintain a variety of postures (kneeling, squatting, crawling, sitting, and standing) for extended periods of time. Maintain all files in a confidential manner

Director of Family Support Professional Development/Training Offered:
Hold and maintain certifications in CPR/First Aid & Safety-Care 20 hours of training (both live and online) annually as assigned by supervisor.
Pay:
$27.00 - $30.00 per hour
