Addictions Counselor - Grace Medical Center - A.C.T. Summary
JOB SUMMARY
Counsels substance use disorder patients, and provides therapeutic interventions and treatment to facilitate recovery. Identifies and assesses patient needs based on referral information and comprehensive behavioral assessment. Completes or reviews comprehensive behavioral assessment and other evaluation tools with patient. Provides ongoing assessment of the patient throughout the course of treatment and modifies recovery plan accordingly. Provides substance use disorder treatment to patients. Orients new patients to program rules and procedures and completes additional admission paperwork as necessary. In collaboration with multidisciplinary treatment team, develops initial and ongoing recovery plan with patient based on comprehensive behavioral assessment, patient input, and other relevant information. Counsels individuals and groups on both a scheduled and a crisis intervention basis to achieve treatment goals and objectives. Provides clinical interventions for complicated addicted patients who may also have HIV/AIDS, chronic medical problems, and/or a psychiatric disorder. Coordinates care of patient with necessary resources. Establishes contact with outside agencies such as lawyers, probation officers, caseworkers, etc. , as required to facilitate patient recovery. Collaborates with multidisciplinary team including psychiatrists, nurses, outpatient therapists, and addictions counselors for input regarding course of treatment. Refers patients to outside services, when needed. Documents all work as dictated by program policy, regulatory agencies, and insurance carriers. Records all interactions with and about patients including individual and group sessions, phone contacts, changes in goals and objectives, etc. Maintains statistics required for the preparation of monthly reports for internal and external monitoring, such as number of counseling sessions, number of patients, program terminations, etc. Completes all paperwork and on-line submission of clinical information for charge reimbursement in a comprehensive and timely manner.
REQUIREMENTS
Minimum of 1 year of behavioral health counseling experience required; 3+ years of experience preferred. High School Diploma/GED required. Associate's Degree preferred. ADT license in Maryland is required for consideration. Must earn an Associate's Degree and CSC-AD or higher licensure within 2 years of employment. Valid MD Driver's License required and clearance by the LBH Fleet Safety Department to operate vehicles.
