Community Support Specialist - Grace Medical Center - SCMP Crisis Summary
JOB SUMMARY
Responsible for assessing patient needs and linking patients to community agencies and resources as dictated by patient need. Conducts an initial assessment and gathers data relevant to patient needs. Assess patient needs and capabilities in the context of specific referral requests made by the primary clinician. Gathers and documents all necessary information needed for making patient referral, e. g. , proof of patient income, current medication dosages, etc. Coordinates services and resources to meet patients ancillary needs. Links patients with the most appropriate services necessary to meet their individual needs. Assists patients with obtaining and maintaining entitlements and benefits for which they are eligible. Communicates with internal staff, patients, and various community resources to coordinate patient assistance. Maintains contact with community agencies as appropriate to initiate and process referrals for vocational, financial and other social support resources. Creates and maintains current community resource list to include such areas as housing, legal assistance, social services, and other resources that may be needed by the patient population. Provides information and inservice trainings to clinical staff as necessary to share information and referral sources that may benefit the patient population. Maintains records on all patients as outlined by Hospital and Department protocol. Communicates with patients and clinical staff regarding the plan for and progress of securing needed resources.
REQUIREMENTS
Minimum of 1 year experience working in a behavioral health setting required; 3+ years of experience preferred. Bachleor's Degree preferred. Valid MD Driver's License required and clearance by the LBH Fleet Safety Department to operate vehicles.
