SUD Discharge Planner

Pay range: $35.20 - $41.15 per hour, depending on years of experience Centurion is proud to be the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Part-time Weekend Day Shift Discharge Planner to join our Substance Use Disorder (SUD) team at Western Correctional Institution located in Cumberland, Maryland. The SUD Program Discharge Planner is responsible for coordinating a patient's release from the substance use program upon release or leave to another facility. This person is responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return patients to referral source at the time of discharge.

Shift:
