We're seeking a passionate Behavioral Health Practitioner to serve as a Co-Occurring Disorder Specialist on our Assertive Community Treatment (ACT) team. This role is ideal for someone who thrives in a collaborative environment, values variety in their day, and is committed to meeting people where they are in their recovery journey. Why This Role Stands Out This isn't a desk-bound position or a predictable caseload. As a Co-Occurring Disorder Specialist, you'll be part of a multidisciplinary ACT team delivering integrated, person-centered care in real-world settings. You'll play a key role in supporting individuals with both severe and persistent mental illness (SPMI) and substance use disorders—helping them move toward stability, recovery, and independence. What You'll Experience True Team-Based Care - Work side-by-side with clinicians, nurses, psychiatry, case managers, and specialists in a fully integrated team model. Shared caseload, shared mission. Variety in Your Day - From substance use assessments and motivational interviewing to field-based outreach and care coordination, no two days look the same. Integrated Dual Disorder Treatment - Deliver evidence-based care that addresses both mental health and substance use—meeting clients at their stage of change. Community-Focused Work - Provide services in homes, workplaces, and community settings where real-life change happens. Purpose-Driven Impact - Build meaningful relationships and support clients through all stages of recovery, celebrating progress along the way. What You'll Do Implement individualized treatment plan services under clinical supervision Complete IDDT assessments and maintain accurate, timely documentation Assess and track stages of change and treatment progression Utilize motivational interviewing and outreach strategies Apply cognitive behavioral techniques and relapse prevention strategies Coordinate care with internal teams and external providers Deliver services aligned with evidence-based practices for co-occurring disorders What You'll Need Meet qualifications for a Behavioral Health Practitioner under Minnesota Statute 245i.462, Subd. 17 Training in co-occurring disorders consistent with evidence-based practices Experience in working with individuals with complex mental health and substance use needs Strong communication, organization, and teamwork skills Clear DHS background study Valid Driver's License and acceptable driving record What Will Set You Apart Experience working on an Assertive Community Treatment (ACT) team Experience working in SUD field; residential experience is preferred Experience with motivational interviewing, CBT, and relapse prevention models Bachelor's or degree in Psychology, Social Work, Marriage & Family Therapy, or related field with experience with SUD, MI/CD and co-occurring disorders Licensed Alcohol and Drug Counselor (LADC) in Minnesota is preferred Schedule Monday - Friday | 7:30 AM - 3:30 PM Optional shared after-hours and weekend on-call rotation opportunities available with stipends
Compensation Behavioral Health Practitioner:
