Job Description

Job Summary We are seeking a dedicated and energetic Qualified Behavioral Health Professional (QP) to join our dynamic healthcare team. In this vital role, you will provide comprehensive behavioral health assessments, develop individualized treatment plans, and deliver compassionate mental health counseling to diverse patient populations. Your expertise will support inpatient, outpatient, and post-acute care settings, ensuring patients receive the highest quality of mental health services. Responsibilities Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, utilizing your knowledge of physiology and medical terminology. Develop and implement personalized treatment plans aligned with patient needs, incorporating evidence-based practices in behavioral health and addiction counseling. Collaborate with multidisciplinary teams across hospital departments such as
ICU, PICU
(Pediatric Intensive Care Unit), emergency medicine, and trauma centers to coordinate patient care effectively. Manage discharge planning and case management activities to ensure seamless transitions from inpatient to outpatient or post-acute care settings. Maintain accurate medical records in compliance with HIPAA regulations, documenting intake, utilization management, and patient progress notes diligently. Participate in utilization management activities by evaluating patient eligibility for services and supporting appropriate resource allocation. Provide patient service through empathetic communication, active listening, and culturally sensitive approaches tailored to pediatric and adult populations. Skills Proven experience working in hospital environments, including Level I Trauma Centers or Level II trauma centers. Strong background in acute care settings such as ICU, emergency medicine, or inpatient units with familiarity in hospital medicine protocols. Knowledge of behavioral health practices related to mental health counseling, addiction treatment, and social work principles. Experience with medical records management, discharge planning, diagnostic evaluation, and case management processes. Ability to perform comprehensive patient assessments utilizing physiology knowledge and medical terminology expertise. Familiarity with outpatient clinic operations and post-acute care processes for seamless patient transitions. Excellent communication skills combined with a proactive approach to patient care coordination within managed care frameworks. $15.00 - $20.00 per hour
