Prairie St. John's mail_outline Get future jobs matching this search Login or Register Job Description Responsibilities PRN Outpatient Licensed Addiction Counselor! Prairie St. John's is seeking an outpatient adult licensed addiction counselor to work with adults in our PHP and IOP levels of care (partial hospitalization program and intensive outpatient). This position is Monday through Friday during regular business hours at out St. Genevieve's Place building on our campus. There may be limited opportunities to work with children and adolescents in outpatient PHP to assist with clinician coverage. The individual in this role will provide therapy in a group setting. The Licensed Addiction Counselor (LAC) evaluates patients for chemical dependency history and uses this information to document signs; symptoms; physical, mental, emotional, social, and spiritual implications; and treatment recommendations. The LAC may be asked to speak with other providers and insurance companies to share their observations and recommendations; as well as re-evaluate patients to determine progress at each level of care. The LAC develops a plan of treatment; shares the recommendations with members of a multi-disciplinary team for feedback; implements feedback and then implements the treatment plan within the established time frame. The LAC monitors the patient's adherence to the plan and determines if short term goals are being met. The LAC provides group therapy and CD education based on accomplishment of objectives identified in treatment plan. This may include individual and family therapy if ordered. The LAC is responsible for coordinating care with referral sources and other providers, as well as treatment and discharge planning. Thorough documentation of assessments, evaluations, therapy notes, contacts made must be made accurately and timely. The LAC has a professional duty to warn in cases of child and vulnerable adult protection.
