Job Summary:
Professional individual who provides support to LifeConnections Family Support staff, birth, foster, and adoptive families in-home & in the community. Serving as a positive and supportive role model for LifeConnections staff, as well as youth and young adults with extensive clinical needs and/or developmental disabilities, and their families. Provides transportation, supervised visitation, parenting education, and promotes positive child-parent/child-sibling relationships. Links children and their caregivers to community resources needed to support the reunification process.
Essential Duties & Responsibilities:
Coordinates visitation(s) between children and their families, supervises and supports visitation sessions as directed by DCYF and/or DJJS, and appropriately documents observations of the visits. Work directly with families that have been challenged with mental illness, substance abuse, and/or developmental delays and have been affected by abuse and neglect. Introduce interventions while working to achieve treatment plan goals and objectives. Connect families with community resources local to them that offer support and promote a safe and healthy lifestyle. Supervises and transports clients during assigned hours, this may include, job search, medical appointments, weekly recreation, shopping trips, and other necessary trips. Reinforce appropriate client behavior through direct feedback, role-playing, and behavior modification techniques. Documentation and timely submission of all contact notes, and all other client documentation deemed necessary by the organization. Attends all team meetings specific to the client. Assist in the collecting, completion, and submission of case management related paperwork
Essential Skills & Abilities:
Manages stress well Excellent communication skills with adults and children Knowledge of childhood development Knowledge of substance abuse/signs of relapse Knowledge of domestic abuse relationships and its effects on children Well organized, dependable, and timely Strong written communication skills. Ability to act appropriately in the event of an emergency. Experience working with youth and/or adults with developmental disabilities is a plus
