Position Overview:
Wellness Clinician-Licensed, LCSW, LPC or LMFT is the lead clinical person for the team, providing Initial Comprehensive Rehabilitation Needs Assessments, skill assessment, rehabilitation planning in conjunction with the team, and clinical supervision for team members. The Wellness Clinician-Licensed, LCSW, LPC or LMFT ensures that clinical interventions are effective, stage-based and tied directly to the individualized recovery goal plans of the people receiving services.
Essential Job FunctionsCreates and Maintains:
A wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning. All services provided will promote wellness and recovery outcomes.

A culture of compliance with internal and external policies, regulations, laws and high ethical standards
CSS Responsibilities:
Lead team member to complete Comprehensive Rehabilitation Needs Assessments (CRNA) in collaboration with the team.

Lead team member to complete, monitor and update the individualized rehabilitation plan/Individual Recovery Plan (IRP) and reviews in collaboration with the team.

Offer services appropriate to meet persons served needs as identified in the Comprehensive Rehabilitation Needs Assessment (CRNA) and included in each individuals IRP.Perform therapeutic rehabilitative skill and resource development as outlined in the CSS RegulationsPerform crisis intervention as indicatedCoordinate and manage services in conjunction with the team.

Oversees the use of Illness Management and Recovery, WRAP, advance directives and other relevant health and wellness curriculum to ensure overall wellness is a key focus in the individuals recovery planPerform evidence-based practices that are appropriate for implementing community support services. This may include Motivational Interviewing, Cognitive Behavioral and Educational Strategies, Trauma Informed Care, Permanent Supportive Housing, IMR, IDDT and Career Services.

Provides mental health education, skill training, and supportive counseling in the home.

Utilizes a clinical skill-set by forging therapeutic relationships with diverse individuals through a trauma-informed and recovery-based lensEnsures that staff provide coordination of referrals and linkages to housing, mental health, substance abuse; medical and various social service providers as needed for the individual.

Appraisal and assessment of, but not limited to, individuals daily living skills, and teach individuals needed skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy and ways to maintain housing.

Consult and refer to other services beyond the scope of CSS.Provide clinical supervision and consultation to team via individually and/or team supervision, and for crisis management.

Seek out own supervision at least monthly from supervisor.

Monitors symptom assessment and management, medication monitoring, medication education and side effects.

Provides support to team when persons served with acute psychiatric needs whose circumstances and conditions require rapid intervention.

Technical assistance and education to members of the team in reference to best clinical practices. Utilizes best practices materials and strategies for individual and group interventions, in conjunction with the team.

Performs other duties as requiredFully licensed clinical staff with supervisor credential will provide clinical supervision as assigned for provisionally licensed Bridgeway employees toward full clinical licensure. Fully licensed clinical staff who have not acquired the supervisor credential and are eligible will obtain the supervisor credential.
Requirements:
Education/Certifications:
Masters degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program.

Practitioner of the healing arts licensed in New Jersey required (LCSW or LPC) A licensed marriage and family therapist (LMFT), who shall possess a Master's degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program. In addition to a Master's degree, licensed marriage and family therapists must have one-year experience in a community behavioral health setting or be certified as a psychiatric rehabilitation practitioner (in lieu of the one year of experience in community behavioral health setting
Experience:
Minimum of one-year experience working with persons with mental health disabilities preferred
Drivers License, Vehicle:
