Job Description The Bereavement Counselor is responsible for assessing the potential needs of grieving community members and creating opportunities for remembrance. The incumbent will provide one-on-one, individual counseling sessions, group sessions, leadership for bereavement events, and promote people through the grief process. To excel in the role, the Bereavement Counselor needs to be passionate about delivering compassionate and empathetic support to variety of people.
Responsibilities:
Counseling Services:
Provide individual and group counseling sessions to people coping with grief and loss due to the death of a loved one. Create a supportive and empathetic environment for clients to explore their feelings, process their grief, and develop coping strategies. Assessment and Treatment Planning:
Conduct comprehensive assessments of clients' emotional, psychological, and social needs related to grief and loss. Develop personalized care plans based on clients' strengths, preferences, and goals. Education and Support:
Offer psychoeducation to clients and their families about the grieving process, common reactions to loss, and coping mechanisms. Provide guidance and support to help clients navigate challenges such as guilt, anger, and feelings of isolation. Collaborative Care:
Collaborate closely with interdisciplinary team members, including social workers, chaplains, nurses, and volunteers, to ensure holistic care and support for patients and families throughout the bereavement process. Participate in team meetings, case conferences, and care planning sessions to coordinate care and address clients' needs. Documentation and Record-keeping:
Maintain accurate and confidential records of counseling sessions, assessments, treatment plans, and client progress using our hospice's electronic health record system. Ensure compliance with regulatory standards and organizational policies regarding documentation and record-keeping. Community Outreach:
Participate in community outreach efforts to raise awareness of bereavement services and resources available through our hospice. Represent the hospice at community events, workshops, and support groups to promote our mission and services. Any other duties as assigned Qualifications:
Requires a Bachelor's degree in counseling, psychology, social work, or a related field. Previous experience in counseling required. Previous experience in grief counseling, bereavement support, or hospice care preferred. Clinical skills and knowledge of therapeutic techniques for addressing grief and loss. Excellent interpersonal and communication skills, with the ability to establish rapport with clients and provide compassionate support. Cultural competence and sensitivity to the diverse needs and backgrounds of clients in a rural community setting. Ability to work independently as well as collaboratively within a multidisciplinary team. Commitment to the mission and values of the hospice, with passion for providing quality end-of-life care support. Proficiencey in computer skills, including experience with scheduling software, database management, and Google Workspace. Must possess outstanding presence, self-confidence & enthusiasm and be comfortable engagin with all levels of stakeholders. Must be capable of performing the essential functions of this job, with or without reasonable accomodations. Valid driver's license and reliable transportation.
