Job Description

STATEMENT OF THE JOB
The Mobile Crisis Coordinator oversees the CCBHC crisis response for individuals experiencing mental health crises in the community, providing direct crisis intervention, assessments, safety planning, and linkage to appropriate services, while ensuring the process operates effectively and adheres to clinical standards and protocols within the CCBHC model; often requiring on-call availability and collaboration with other community agencies.
RESPONSIBILITIES AND TYPICAL WORK ACTIVITIES
Managing incoming crisis calls, supporting Same Day Access clinicians. Coordinating mobile responses with law enforcement or other emergency services when necessary. Travel between multiple service sites, to the homes, or residences or persons served, and within the community. Perform a range of clinical services related to the assessment, care, and treatment of individuals experiencing crises, in the school, home, community, or in the outpatient clinic setting. Conducting crisis assessments, developing safety plans, providing immediate intervention strategies, and stabilizing individuals in crisis situations. Assess the need for inpatient stabilization and facilitate admissions as appropriate. Identifying and connecting individuals in crisis with appropriate community resources, including community providers, support groups, and housing assistance. Responsible for the development of policies, procedures and workflows that govern the expectations of service delivery and subsequent documentation. Ensuring accurate and timely documentation of crisis interventions, assessments, and client interactions within the electronic health record in accordance with NYS regulations. Participate in multidisciplinary team meetings. Monitoring team performance, identifying areas for improvement, and implementing quality assurance and improvement measures to maintain high standards of care. Building and maintaining relationships with local community partners, County DCS, law enforcement, hospitals, 988 providers and other agencies to facilitate seamless crisis response. Promote agency goals and positive agency image in the community Ability to develop and deliver community-based presentations representing FCCS and the Mobile Crisis Service.
PREFERRED QUALIFICATIONS
Graduation from a regionally accredited or NYS recognized college or university with a Master's degree in social work, counseling, psychology, or a related field with an active clinical NYS license (LCSW, LPC, LMHC) Minimum two (2) years experience working with adults and/or children with MH/SUD needs Extensive experience in crisis intervention and mental health assessment Expertise in addressing trauma and promoting recovery of children, adolescents, adults and older adults Experience working with children, families and adults who experience complex emotional and behavioral difficulties which may present in crisis Strong understanding of the CCBHC model and its service delivery principles Excellent communication and interpersonal skills to effectively collaborate with clients, families, and community partners Ability to work independently and as part of a team in a fast-paced environment Proficiency in electronic health records and crisis management systems Must have a valid driver's license, reliable operating transportation
