Our organization provides client-centered outpatient services rooted in compassion, professionalism, and evidence-based care. We specialize in serving individuals in group living environments with mental health and past substance use challenges within nursing homes and assisted living facilities.
Job Summary:
We are seeking a compassionate and skilled Licensed Counselor to join our outpatient mental health team. The LSW/LPC will provide high-quality mental health services, including individual psychotherapy and group therapy, within community-based group living environments. This role is ideal for a dedicated clinician who is passionate about serving diverse populations with behavioral health needs, including older adults and individuals with chronic mental illness or co-occurring disorders.
Key Responsibilities:
Deliver individual psychotherapy and group therapy sessions in group homes, assisted living facilities, and skilled nursing facilities. Conduct psychosocial assessments and develop individualized treatment plans tailored to client needs. Collaborate with facility staff, medical providers, and other care team members to ensure integrated and coordinated care. Monitor and document client progress, adjusting treatment plans as necessary. Maintain accurate, timely, and professional clinical documentation in accordance with agency, state, and federal guidelines. Provide crisis intervention and support when needed. Participate in regular supervision, clinical meetings, and ongoing professional development. Advocate for clients and assist in accessing appropriate community resources and services.
Qualifications:
Current and valid LSW/LPC license in the state of Ohio. Preferably 1 year of experience providing therapy in behavioral health, preferably in group living or long-term care settings. Strong clinical skills in individual and group psychotherapy. Knowledge of mental health diagnoses, treatment modalities, and evidence-based practices. Excellent communication, organizational, and interpersonal skills. Valid driver's license and reliable transportation (travel between facilities required).
Preferred Qualifications:
Experience working with older adults, individuals with serious mental illness, or clients with cognitive or physical disabilities. Familiarity with electronic health records (EHR) and Medicaid/Medicare documentation requirements.

Facilitate group therapy sessions and engage clients in therapeutic activities to promote socialization and skill development. Document client progress and behavior accurately, maintaining confidentiality in compliance with HIPAA regulations. Collaborate with multidisciplinary teams to develop individualized care plans that address the unique needs of each client. Skills Experience working with individuals with disabilities in a direct support capacity is essential. Knowledge of behavioral health practices and principles, including familiarity with Applied Behavior Analysis techniques. Strong communication skills, both verbal and written, to effectively interact with clients, families, and team members. Ability to work collaboratively within a clinical setting while maintaining professionalism at all times. Compassionate approach towards clients dealing with PTSD or other mental health challenges. Flexibility to adapt to changing situations and client needs while maintaining a positive attitude.
