Job Description

MISSION:
To provide compassionate care to all; healing and learning together.
PURPOSE OF POSITION
The primary purpose of the Behavioral Health Consultant is to provide coordinated behavioral health services and support to patients accessing medical care at My Community Health Center (MCHC). Care is delivered in a team-based framework within a fully integrated behavioral health and medical model. This position reports to the Clinical Director for Behavioral Health.
RESPONSIBILITIES AND EXPECTIONS

JOB REQUIREMENTS
1. Screening, assessment, and engagement a. Support excellent adherence to behavioral health screening recommendations as established by the Health Resources and Services Administration (HRSA) and reported in the Uniform Data System (UDS) for quality assurance. b. Follow up on abnormal depression screenings with repeat screening and assessment per UDS guidelines c. Perform screening and follow up for behavioral health needs in the primary care setting including: i. Depression ii. Anxiety iii. Substance use disorder iv. Domestic violence v. Suicide risk d. Assess patient needs based on findings of routine screening evaluations e. Work with an interdisciplinary team to follow up on patient needs and coordinate care 2. Treatment Planning, Collaboration, and Referral a. Determine the level of care needed for patients with identified behavioral health needs including crisis intervention and stabilization when indicated b. Engage with the interdisciplinary team to develop an appropriate treatment plan including referral to internal and external partners for further evaluation and management as indicated 3.
Counseling:
Individual, Family, and Group a. Provide counseling to individual patients and families when indicated during provision of primary care services b. Individual and group counseling and motivational interviewing for issues impacting overall patient wellness, including: i. Smoking cessation ii. Alcohol cessation iii. Substance use disorder treatment iv. Health related behaviors such as health diet and physical activity c. Individual counseling for mental health needs including: i. Depression ii. Anxiety iii. Grief iv. Adjustment disorder 4. Professional and Ethical responsibilities a. Maintain accurate and timely patient records with chart sign off within 48 hours of service delivery b. Act as a mandated reporter within Ohio Revised Code guidelines c. Maintain patient confidentiality in communication and record keeping d. Maintain clinical supervision requirements within the scope of your licensure

  • Adhere to professional and ethical behavior standards of the healthcare industry
QUALIFICATIONS
  • Graduate of approved school with degree in social work or related field
  • Current LISW licensure in the State of Ohio
  • Demonstrated excellence in interdisciplinary collaboration
  • Previous FQHC experience preferred
  • At least one year's experience in an outpatient setting preferred
WORKING CONDITIONS
  • Monday through Friday 8:00am - 4:30pm with occasional late nights or varying hours as required by the job. Occasional weekends and off shifts.
  • Must be able to meet all physical and mental health requirements of the position and maintain an excellent attendance record.
  • Must adhere to policies and procedures.
  • Must fulfill position description requirements.
  • Is subject to frequent interruptions during performance of duties.
  • Must use a computer daily.
