In this vital role, you will provide high-quality, coordination of evidence-based mental health assessments, develop personalized care plans, and deliver compassionate support to promote resilience and independence. Your proactive approach and dedication will help create a safe, supportive environment that fosters growth, healing, and empowerment for all clients. Duties Coordinate comprehensive mental health assessments to evaluate clients' needs, strengths, and treatment goals. Develop individualized care plans in collaboration with clients, families, and multidisciplinary teams to ensure holistic support. Deliver and coordinate evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), motivational interviewing, or other modalities suited to client needs. Monitor client progress regularly, adjusting treatment strategies to optimize outcomes and ensure continuous improvement. Educate clients and their families about mental health conditions, coping strategies, and available resources to foster understanding and self-management. Maintain accurate documentation of assessments, treatment plans, progress notes, and incident reports in compliance with organizational policies. Collaborate with healthcare providers, social workers, case managers, and community agencies to coordinate care and facilitate seamless support networks. Requirements Bachelor's degree or higher in psychology, social work, counseling or related mental health field; master's degree, licensure or certification preferred. High School Diploma applicants with 3-5 years or more experience will be considered. Proven experience working with individuals experiencing mental health issues in clinical or community settings. Strong knowledge of evidence-based therapeutic techniques and mental health assessment tools. Excellent communication skills with the ability to build rapport and foster trust with diverse populations. Ability to work independently while effectively collaborating within a multidisciplinary team. Demonstrated commitment to ethical standards, confidentiality, and client-centered care. Flexibility to adapt to changing client needs and organizational priorities. Join us in making a meaningful difference!
