We are seeking a compassionate and dedicated Mental Health Counselor - LSW/LPC/MFT to join our interdisciplinary behavioral health team. The ideal candidate will provide clinical mental health services, psychosocial assessments, counseling interventions, and care coordination for individuals with behavioral health needs. This role requires strong clinical judgment, effective communication skills, and the ability to work collaboratively within a multidisciplinary environment.
Responsibilities:
Conduct mental health screenings, psychosocial assessments, and risk evaluations, including suicide risk assessments within scope of practice and licensure guidelines. Provide individual and group counseling interventions based on treatment plans and clinical recommendations. Deliver psychosocial education and therapeutic group sessions. Monitor client progress toward treatment goals and document outcomes appropriately. Collaborate with interdisciplinary team members to support individualized treatment planning. Complete treatment assessments, transfer summaries, discharge planning, and related clinical documentation. Maintain timely and accurate electronic health records, progress notes, and reports in accordance with organizational policies. Coordinate care and maintain communication with families, healthcare providers, and community agencies to support continuity of care. Make appropriate referrals to community resources and support services. Participate in clinical supervision, staff meetings, in-service trainings, and professional development activities. Maintain professional conduct and ethical standards at all times. Perform additional duties as assigned.
Pay:
