PrimaryOne Health is currently seeking Family Support Specialist.
Essential Job Responsibilities:
Responsible for initiating and maintaining regular and long-term (up to three years) contact/support with families. This activity will occur primarily within the family's home; each visit should last for at least one hour. Develop interventions that are family-centered, strength-based, and directed at: establishing a trusting relationship; assisting in strengthening the parent-child relationship; assisting parents in improving their skills to optimize the home environment; improving the family support system; and increasing the family's ability to problem solve and assume the role of advocate for themselves and their children. Conduct assessments with families to determine any social determinants /child development/needs. Responsible for assisting the family in establishing goals and a plan for accomplishment of those goals, as well as the assessment of the normal growth and development of the target child. Conduct follow-up calls or visits to explain and enroll individuals in appropriate resources; educate on how to use insurance. Identifying and referring for contact/appointments at other supportive agencies/social resources, including health care appointments and advocating on their behalf with external agencies and institutions. Schedule appointments and conduct reminders for patients. Ensure appropriate resources are available for attending appointments. Follow-up with clients by letter, phone call, or home visit on missed appointments or referral. Document client needs, action plan, and follow up intentions in EHR. Adhere to confidentiality rules and regulations. May be responsible for attending various community meetings and/or events as a PrimaryOne Health representative as assigned. Occasional travel to various PrimaryOne Health center sites required. Attendance, promptness, and the ability to pay attention to detail. Must have a professional demeanor, show strong initiative, be flexible, friendly, cooperative, have exceptional communication skills and an ability to engage others (co-workers, patients, customers, students, and community partners). Other duties or special projects as assigned. Successful candidates will have a high school diploma or equivalents. Must obtain a Certified Home Visit Provisional License within the first 3 months of employment followed by the Certified Home Visit Full License from Ohio Department of Health within a year of employment. Requires 1-3 years of related experience in working with or providing services to children and their families. Work is generally independent and collaborative in nature. Strong knowledge of health disparities and health issues of FIMR. Knowledge of some OB/GYN medical terminology and infant and child development. Basic computer skills. Experience working with diverse community groups.
