Youth Psychiatric Transition Specialist

The Youth Psychiatric Transition Specialist will be focused solely on establishing relationships with local psychiatric facilities in order to improve the timeliness and length of engagement in Care Coordination. They will serve as the primary liaison between inpatient psychiatric hospitals and the Care Management Entity (CME) to ensure continuity of care for youth transitioning from higher levels of psychiatric care to community-based services. Maintain a consistent physical and/or virtual presence within inpatient psychiatric units to ensure real-time awareness of admissions, treatment planning, and anticipated discharges. This position has the potential of evening hours.

Required Qualifications:

•Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board as a LSW, LISW, LPC, or LPCC• A. Experience working with youth with serious emotional disturbance, psychiatric conditions, or complex behavioral health needs. B. Demonstrated ability to navigate hospital systems and collaborate with multidisciplinary clinical teams. C. Strong relationship-building, communication, and organizational skills. D. High emotional intelligence and the ability to build trust with hospital staff working in high-pressure environments. E. Proactive problem-solving skills, including the ability to identify system barriers and present practical solutions. F. Ability to work independently while maintaining alignment with CME supervision and team structures. G. Ability to utilize electronic health records, tracking systems, and agency-issued technology.

Education and Experience:

A. Relevant experience in behavioral health, care coordination, hospital discharge planning, or a related human services field; B. Bachelor's degree plus 2 years of relevant experience; or C. Master's degree plus 1 year of relevant experience. D. Experience working within hospital, psychiatric, behavioral health, or care management systems preferred. E. Knowledge of Ohio behavioral health systems, Medicaid services, and community-based resources are preferred. F. Valid driver's license, reliable transportation, and ability to travel within the service area. G. Successful completion of required background checks and pre-employment screenings in accordance with Ohio regulations.
