Primary Counselor Horizon House - 3.4 Philadelphia, PA Job Details Full-time 22 hours ago Benefits Commuter assistance Paid holidays Health insurance Dental insurance Tuition reimbursement Paid time off Opportunities for advancement Retirement plan Qualifications Driver's License Working with individuals with mental health conditions Full Job Description We are seeking a Full-Time Primary Counselor to work in our Behavioral Health Services Division. To provide care and supervision of all assigned clients to ensure a safe, healthy, secure, and pleasant environment, which promotes growth and development, and facilitates client growth and development toward independence in self-care, daily living skills, socialization, and community living, participate in the development of the individual service plans and related documentation. Other Responsibilities, but not all-inclusive: Provide guidance and support to all clients; teach and reinforce skills in mental health symptom reduction, daily living skills, routine medical care, and personal hygiene. Participate in developing individual service plans for assigned clients. Conduct a weekly counseling session with all assigned clients to review progress toward treatment plan goals. Record a weekly progress note of this session in the DAP format.
Qualifications:
High School Diploma, Associate's degree preferred in the social sciences or a health care-related field, and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required. Good communication skills, both oral and written. Ability to work independently and as part of a team.
Scope:
Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents. Possess manual dexterity and fine motor skills. May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs). May require crisis intervention in emergencies. May be expected to report to work as needed during emergencies, including inclement weather.
