Apartment Treatment Housing Counselor - Office of Mental Health (OMH) Full-time hours. Schedule is as follows : Sunday 12-8p (Hybrid), Monday 10a-6p (Hybrid), Tuesday 9a-5p (In-office), Wednesday 12-8p (In-office), Thursday 10a-6p (In-office); off

In this role as an Apartment Treatment Housing Counselor, you will: Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services. Monitor, train and assist residents in all programs in household maintenance activities to ensure a clean, safe and homelike environment. Directly participate in household maintenance activities when required. Provide training, supervision and assistance to residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities. Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals. Conduct fire safety training activities. Document interactions with residents in accordance with agency policies and standards. Meet weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals. Assist residents in procuring and maintaining financial entitlements. Monitor medications as required. This position as Apartment Treatment Housing Counselor requires: High school diploma or equivalent. Must have transportation and be able to travel through Nassau and Western Suffolk County.
