$56,000- 66,000 per year

We're inviting an experienced and mission-driven Behavioral Health Counselor to join our Integrated Care at Home team. Relationship-Based Care:

- Care is provided in private homes and other care settings, allowing you to meet patients where they are—physically, emotionally, and medically.
- What You'll Do
- Conduct in-home assessments of mental, emotional, and behavioral health.
- Collaborate with multidisciplinary team to develop treatment goals.
- Deliver full range of psychological and counseling services
- Deliver evidence-based, clinical interventions.
- Develop and review treatment plans/discharge/referral care coordination.
- Provide services in various clinical settings, in-person, off site, or virtually.

Qualifications Required:
- Current Licensed Master Social Worker in the State of Michigan or LLMSW with supervision available.
- Excellent communication & relationship-building abilities with patients, families, and teams
- Valid driver's license with ability to travel throughout Berrien, Cass, and Van Buren counties
Preferred:
