What You'll Do Comprehensive Outpatient Care Conduct mental health assessments and develop individualized treatment plans. Provide outpatient therapy to adults and/or children. Deliver crisis intervention and behavioral stabilization as needed. Maintain a structured caseload of one patient per hour to ensure quality care. Integrated Team Collaboration Work side-by-side with Primary Care Providers within our MindPATH model. Collaborate with care managers and interdisciplinary teams. Connect patients with appropriate community and behavioral health resources. Clinical Documentation & Monitoring Maintain accurate treatment plans and progress notes. Monitor patient outcomes and adjust interventions as needed. Prepare discharge planning and case summaries.

What You Bring Required:

Master's Degree in Social Work, Counseling, or related field. Active LPC or LMSW license. Minimum 3 years of experience providing outpatient behavioral health services (preferred) Experience with assessments, treatment planning, therapy, and crisis intervention.
