in Virginia Beach, Norfolk, Chesapeake, Portsmouth, Suffolk, and the Eastern Shore, VA!

Status:

Full-time, permanent position (40 hours) Standard working hours: 8am to 5pm

EST, M-F

The position requires both in‑person, face‑to‑face assessments (1-3 times per week) and remote telephonic assessments of adult and pediatric members with Serious Mental Illness (SMI), Severe Emotional Disturbance (SED), and Substance Use Disorders (SUD) throughout Virginia Beach, Norfolk, Chesapeake, Portsmouth, Suffolk, and the Eastern Shore, as needed.

Candidate must reside in Virginia Beach, Norfolk, Chesapeake, Portsmouth and Suffolk, VA.

Job responsibilities:

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses. Education Must possess a degree in Health & Human Services or one of the following related
