This role involves coordinating services, conducting home visits, linking families to community resources, and ensuring compliance with state and federal early intervention regulations. Key Responsibilities Conduct initial and ongoing family assessments to identify strengths, needs, and goals. Develop, implement, and monitor individualized family service plans (IFSPs) in collaboration with multidisciplinary teams. Schedule and facilitate home- or community-based visits, ensuring services are timely and family-centered. Link families to community resources, support groups, and early childhood programs. Provide training and coaching to caregivers on developmental strategies and interventions. Track caseload data, maintain accurate records in state reporting systems, and ensure confidentiality. Coordinate with therapists, educators, health providers, and other stakeholders to streamline service delivery. Assist families in navigating insurance, Medicaid, and social service applications. Participate in team meetings, case conferences, and professional development activities. Monitor program outcomes and contribute to continuous quality improvement efforts. Required Qualifications Bachelor's or Master's degree in Social Work, Psychology, Education, Counseling, Child Development, Nursing, or a related human services field. Valid driver's license, reliable transportation, and current vehicle insurance. Minimum of two years of experience in case management, early intervention, or related family services. Familiarity with IDEA Part C early intervention requirements and Louisiana EarlySteps policies. Proven ability to manage multiple tasks, prioritize deadlines, and work independently. Preferred Qualifications Experience working with children birth to three and their families in home- or community-based settings. Bilingual proficiency in English and Spanish or another language common in Louisiana communities. Certification in early childhood development or family support (e.g., CDFC, IFSP training). Knowledge of local community resources, non-profits, and social service agencies. Knowledge, Skills, and Abilities Strong written and oral communication, including report writing and presentation skills. Empathy, cultural competence, and ability to establish rapport with diverse families. Excellent problem-solving and critical-thinking skills. Proficiency with Microsoft Office Suite and web-based case management systems. Ability to work collaboratively in a multidisciplinary team and independently in the field. Flexibility to travel within assigned region and adapt to varying family schedules. Work Environment and Physical Requirements In-person position requiring travel for home and community visits. Ability to sit, stand, kneel, and lift up to 30 pounds for brief periods. Occasional work in varied home environments and community settings.
