About Healthy Families Sierra County Healthy Families Sierra County (HFSC) is a community-based intensive home visiting program affiliated with Healthy Families America® (HFA). Our mission is to promote child well-being and prevent the abuse and neglect of children through relationship-centered, voluntary home visiting services. We partner with families in Sierra County from pregnancy through a child's fifth birthday, meeting families where they are with cultural humility, equity, and deep respect for lived experience. Position Summary The Family Support Specialist (FSS) provides trauma-informed, culturally responsive, and relationship-based home visiting services to pregnant people and families with young children in Sierra County and surrounding rural areas. Working within the Healthy Families America (HFA) model, the FSS builds long-term, trusting partnerships with families over the course of three to five years, supporting parenting practices that promote healthy child development, family well-being, and connection to local resources. Home visits occur primarily in the family's home, typically lasting one hour, and are conducted in-person and virtually based on family need and preference. The FSS helps families identify goals, complete assessments and screenings, reduce risk, and build protective factors through consistent, strengths-based support and collaboration. Essential Responsibilities Establish and maintain regular, long-term contact with assigned families, providing family-centered home visiting services per HFA standards. Build trusting, respectful relationships with families to support parenting confidence, reduce isolation, and promote parent-child attachment. Complete the Family Resilience and Opportunities for Growth (FROG) assessment and collaborate with families to create individualized Family Service Plans. Administer and track developmental screenings (ASQ, ASQ-SE), depression screens (EPDS/PHQ-9), and other tools per protocol. Support families in accessing services such as health care, early childhood programs, behavioral health, housing, education, and employment. Document all visits, assessments, referrals, and service plans accurately and promptly in the data management system. Collaborate with community partners and internal staff to support coordinated services for families. Participate in weekly reflective supervision, group meetings, and ongoing professional development. Maintain professional boundaries, confidentiality, and ethical standards at all times. Uphold fidelity to the Healthy Families America model and ensure alignment with all policies and procedures. Qualifications Education:
High school diploma or equivalent required Associate's or Bachelor's degree in social work, early childhood, human development, or related field preferred
Experience:
Experience working with or providing services to young children and families Knowledge of infant/child development, parent-child interaction, and family dynamics Experience working with diverse and rural populations preferred
Preferred:
Bilingual (English/Spanish) — strongly preferred and eligible for a 5% language incentive per ECECD guidance Experience in home visiting, early childhood, or case management Infant Mental Health Endorsement (IMH-E®), if available in New Mexico Certified Community Health Worker (CHW) or Home Visitor Certificate Work Conditions Frequent travel required within Sierra County and surrounding areas; mileage reimbursement provided Flexible scheduling required, including occasional evening hours to meet family needs Must pass background checks and complete required trainings per HFA and state guidelines Reliable transportation, valid driver's license, and proof of insurance required Compensation Starting salaries follow the HFSC Compensation Schedule (effective 2/18/2026) and meet or exceed the minimums established in the New Mexico ECECD Unified Wage Scale & Career Lattice (December 2025), under which the FSS role is categorized as an Early Childhood Professional - Home Visitor. A 5% incentive is available for Dual Language Certification and Trauma Informed Care Certification per ECECD guidance. Merit increases and cost-of-living adjustments are separate from the education-based schedule. All increases are contingent on available funding.
