Center for Families and Relationships (CFAR) seeks an Intake and Assessment Therapist who is responsible for conducting comprehensive behavioral health assessments for individuals seeking outpatient mental health services. This role serves as the primary point of clinical contact for new clients, completing diagnostic evaluations, risk assessments, treatment recommendations, and service referrals. The Intake and Assessment Therapist collaborate with multidisciplinary staff to ensure timely access to appropriate levels of care and supports the organization's mission of providing high-quality, person-centered behavioral health services. This position reports to the Site Clinical Supervisor. Key Responsibilities Clinical Assessment & Intake Conduct comprehensive biopsychosocial assessments for new clients seeking mental health services. Maintain productivity of 25 completed intakes per week. Complete diagnostic evaluations and formulate clinical impressions using current diagnostic criteria. Assess presenting concerns, psychiatric symptoms, trauma history, substance use, medical history, family dynamics, social determinants of health, and functional impairments. Conduct suicide risk, homicide risk, self-harm, abuse, neglect, and safety assessments. Determine client eligibility and appropriateness for outpatient services. Develop initial treatment recommendations and level-of-care determinations. Facilitate referrals to internal programs or external community resources as clinically indicated. Collaborate with clients to establish preliminary treatment goals and identify service needs. Obtain necessary medical records for client files, custody paperwork, and initial outreach to PCP. Documentation & Compliance Complete all intake assessments, diagnostic documentation, treatment recommendations, and related records within required timeframes. Maintain accurate, timely, and comprehensive clinical documentation in the electronic health record. Ensure compliance with organizational policies, state regulations, accreditation standards, and payer requirements. Obtain and document informed consent, releases of information, and required intake paperwork. Maintain confidentiality in accordance with HIPAA and professional ethical standards. Care Coordination Coordinate client assignment to appropriate programs and services. Collaborate with front desk to get clients scheduled for ongoing therapy. Communicate assessment findings and recommendations to treatment providers. Facilitate continuity of care through collaboration with internal teams and external providers. Assist clients in accessing psychiatric, case management, crisis intervention, substance use, and community support services as needed. Participate in multidisciplinary team meetings and case consultations Professional Qualifications Must hold a Master's Degree in a psychology Minimum 2 years of post-master's clinical experience (or completion of CFAR internship) Experience conducting diagnostic assessments and risk evaluations preferred Background in outpatient or community mental health settings strongly preferred Experience using electronic health record systems
