ACT is a long-term, community-based service model that provides intensive, wraparound support for adults living with serious and persistent mental illness. This interdisciplinary program integrates mental health services, psychiatry, nursing, housing support, crisis response, and case management to help individuals achieve stability and independence in the community. ACT serves individuals who require a higher level of care than traditional outpatient services and emphasizes meeting people where they are to support recovery and long-term success.
Schedule:
1.0 FTE, Flexible days and start/end time, Hybrid schedule.
Location:
2121 Park Ave, Minneapolis MN 55404, and in the community Hiring range: $22.00 - $26.14/ hourly, plus $1,000 hiring bonus for external applicants. Pay is variable on experience and qualifications. Main Job Duties:
Developing positive relationships that support clients in meeting their goals Demonstrating intermediate-level of clinical knowledge and modeling the client's goals, treatment plan and diagnosis Providing crisis intervention and de-escalation for clients experiencing mental health symptoms or crises Providing various skill-building, teaching and/or coaching as it relates to the individual Conducting assessments and treatment plans, plan-of-care reviews and discharge plans. Collaborating with internal program staff to support clients around their treatment goals Documenting observable behaviors and symptoms Required Qualification Bachelor's degree in a behavioral science/related field one-year (2000 hours) experience providing employment services or advanced education that involved field training in vocational services to individuals with mental illness. in lieu Two years or 4,000 hours of supervised experience in the delivery of services to people with serious and persistent mental illness OR Master's Degree in behavioral science or related field one-year (2000 hours) experience providing employment services or advanced education that involved field training in vocational services to individuals with mental illness. Or In lieu of a bachelor's degree in a behavioral science/related field, proof of 10,000 hours (Five Years) of experience in the delivery of services to people with mental illness. one-year (2000 hours) experience providing employment services or advanced education that involved field training in vocational services to individuals with mental illness.
Preferred Qualifications:
Preference will be given to candidates with experience working in Assertive Community Treatment (ACT) programs . Experience in working with clients to develop job skills, navigate job searches, and provide initial support to maintain employment. Such as networking with local businesses and hiring managers to maintain directory of jobs and/or volunteer opportunities Experience working with individuals with co-occurring medical and/or chemical dependency and
