CareHawaiiLP Position Summary:
THIS POSITION IS FOR OUR BHCC, CBCM/ICM, CRISIS, LCRS, AND SICM PROGRAMS ON OAHU, KAUAI, AND HAWAII ISLAND
(BIG ISLAND). Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Peer Specialist to support consumers through CAREs provision of services. CARE Hawaii, Inc. contracts with multiple stakeholders to provide an array of services. The Peer Specialist is a member of a multidisciplinary team, responsible for organizing, coordinating, and monitoring all contract operations under the direction of the Team Lead/Director. The purpose of the Peer Specialist is to provide peer support and coaching to recipients of mental health services for severe and persistent mental illness. The peer specialist provides high expertise that professional training cannot replicate. Peer specialists are fully integrated team members who provide highly individualized services in the community and promote client self-determination and decision making.Position Qualifications:
Minimum Education :
Must have a certification as a Peer Specialist in the State of Hawaii (HCPS), a High School diploma or equivalent preferred, AND demonstrate some evidence of mental health activity in the past two years (i.e. advocacy, volunteer, support group involvement etc.)Preferred References:
at a minimum of three (3) verified professional references attesting to the individual's skills, ability & competency to meet the position duties and responsibilities.
Minimum Field-of-Expertise :
Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness. Must be in recovery for at least one (1) year and maintain mental health as determined by individual treatment planRequired Certification/Registration :
Basic Life Support:
(renew upon it's expiration) o CPR o First Aid o CPI (certification is not required) o Current Peer Specialist Certification (State of Hawaii)Preferred Education/Experience :
Paid or volunteer experience with adults with severe and persistent mental illness and AMHD certification.Job Competencies Required Skills:
Must have the ability to demonstrate in excellent interpersonal skills to effectively work with all levels of personnel and with other agencies. Mature, reliable, and responsible. Must have clear, concise, and empathetic communication skills. Must be culturally sensitive and aware. Must know the basics of ethics, HIPPA standards, and problem solving. Must display sound judgment, a good work ethic, and personal integrity. Must have knowledge of community resources, outreach, mental/behavioral health issues in Hawaii. Ability to relate effectively to individuals with a mental illness, substance abuse issues, co-occurring disorders, and homelessness.Technical Skills:
Basic use of a computer and/or tablet, Microsoft Word Suite, fax machine, telephone, and text and chat features on the telephone or computer.Working Environment:
May be in modern well-lighted, air-conditioned work areas and/or in community-based settings; provides outreach in the community. Indoor/outdoor, open windows or air-conditioned environment; may require some travel, over-time, weekend, and holiday work, working with other Federal and/or State accounting bodies, noise level in the environment may be moderate to noisy. Exposure to infectious diseases is a relatively common risk.Special Demands:
Ability to respond effectively to the varied needs and demands of multidisciplinary team members. Have access to helpful, adequate, competent, and continuous supports and services. Harnesses the active involvement of team members in taking ownership of aspects of program development, including operations, record keeping, staff/client education, treatment planning, and quality assurance activities.Consumers Served :
CARE Hawaii will provide services to any individual who meets the criteria of the program, eighteen (18) years or older who have a serious mental illness (SMI). The treatment of individuals should reflect an understanding of the developmental needs and range of treatment needs for each client.Physical Demands:
1. Sit for long periods of time, use of proper ergonomics, the ability to focus on a computer and/or tablet for long periods of time. 2. Ability to assist with the high volume of calls; ability to communicate effectively both verbally and in writing is essential. 3. Ability to follow instructions and motivation to refer to various rehabilitative groups. Frequent reading of printed materials is essential. 4. Listening skills necessary to effectively respond to consumers. 5. Ability to identify and support the daily interactions and to deal with each situation in a stress-free manner.Duties and Responsibilities:
Essential Functions- Demonstrates a welcoming, empathic, and integrated approach towards all consumers on the team
- Attend and participate in bi-weekly team meetings
- Attend in-service training in techniques and modalities relevant to treatment and rehabilitation; in house as well as, trainings sponsored by
DIVISION
i.e., compliance, fraud, and abuse prevention; and other sponsored training by professional organizations.- Documentation of actual face to face interaction will be charted on progress note within twenty-four (24) hours of the interaction.
- Demonstrates ability to work as a Team player by working with other Members of the case management team, assisting other team members as needed Provide consultation when needed to the treatment team and consumer in their recovery process Utilize specific interventions including Wellness Recovery Action Plan (WRAP), Intentional Peer Supports and Recovery Stories as a means to recognize early triggers and signs of relapse and to assist consumers in meeting their individualized recovery goals Lead as well as teach how to facilitate WRAP and promote Seeking Safety and other evidence-based and/or best practice methods Assist consumers in articulating their personal goals and objectives for recovery Outreach/accompany consumers to ensure the individual is making a successful transition to community integration and is continuing their progress toward recovery goals Assist consumers in building social skills in the community that will enhance job acquisition and tenure Support the consumer in seeking to connect/reconnect with family, friends, significant others and in learning how to improve or eliminate unhealthy relationships Assist consumers in creating their personal recovery plans, such as WRAPs, Declarations for Mental Health Treatment, crisis plans, etc.
