Behavioral Health, Substance Use & Re-Entry Services Hometown Health System Location:

Behavioral Health Services Position Summary Hometown Health System is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide integrated behavioral health and substance use disorder services within our Federally Qualified Health Center (FQHC). This position will focus on serving individuals with co-occurring mental health and substance use disorders, including justice-involved individuals, patients transitioning from incarceration, residential treatment programs, and other high-risk populations. The LCSW will provide individual therapy, behavioral health assessments, substance use counseling, crisis intervention, care coordination, and re-entry support while collaborating with medical, psychiatric, pharmacy, case management, and community partners to improve patient outcomes and reduce barriers to recovery. Essential Duties & ResponsibilitiesClinical Assessment & Treatment Conduct comprehensive psychosocial, behavioral health, and substance use disorder assessments. Develop individualized treatment plans utilizing evidence-based practices. Provide individual counseling for mental health and substance use disorders. Deliver trauma-informed, recovery-oriented, and patient-centered care. Perform risk assessments for suicide, self-harm, substance use relapse, and psychiatric crises. Maintain accurate and timely clinical documentation in the electronic health record (EHR). Substance Use Disorder Services Provide counseling for individuals with opioid use disorder, stimulant use disorder, alcohol use disorder, and other substance use disorders. Support Medication-Assisted Treatment (MAT) programs in collaboration with medical providers.
Utilize evidence-based interventions including:
Motivational Interviewing (MI) Cognitive Behavioral Therapy (CBT) Relapse Prevention Therapy Solution-Focused Therapy Harm Reduction Strategies Assist patients in developing recovery and relapse prevention plans. Justice-Involved & Re-Entry Services Support patients transitioning from county jails, correctional facilities, drug courts, and residential treatment programs. Coordinate discharge planning and community re-entry services. Collaborate with probation officers, parole officers, court systems, and correctional agencies as appropriate. Assist patients in accessing housing, transportation, employment, education, and community resources. Promote continuity of care during transitions between institutions and community settings. Care Coordination & Case Management Collaborate with primary care providers, psychiatric providers, pharmacy staff, peer support specialists, and case managers. Coordinate referrals for higher levels of care including detoxification, residential treatment, intensive outpatient programs, and community services. Address social determinants of health affecting treatment engagement and recovery. Participate in multidisciplinary care team meetings and case reviews. Crisis Intervention Respond to behavioral health crises and urgent patient needs. Conduct safety planning and stabilization interventions. Facilitate referrals for emergency psychiatric evaluation or hospitalization when necessary. Community Outreach & Program Development Participate in outreach initiatives targeting underserved and high-risk populations. Support behavioral health program development and quality improvement initiatives. Assist with implementation of integrated care and population health strategies. QualificationsRequired Master's Degree in Social Work (MSW) from an accredited institution. Current Kentucky Licensed Clinical Social Worker (LCSW) license in good standing. Minimum of two years of clinical experience in behavioral health, substance use treatment, or integrated healthcare settings. Experience providing therapy to individuals with co-occurring mental health and substance use disorders. Knowledge of trauma-informed care and evidence-based treatment modalities. Strong assessment, crisis intervention, and care coordination skills. Ability to work effectively within a multidisciplinary healthcare team. Preferred Experience working in an FQHC, community mental health center, correctional setting, residential treatment program, or substance use treatment program. Experience serving justice-involved populations. Familiarity with Kentucky Medicaid behavioral health programs and Targeted Case Management services. Experience supporting Medication-Assisted Treatment (MAT) programs. Training in Motivational Interviewing, CBT, DBT, EMDR, or other evidence-based interventions. Bilingual skills preferred.

$65,000 - $85,000 annually, based on experience and qualifications.

$70,000.00 - $90,000.00 per year
