Job Description
POSITION ACCOUNTABILITY
This position is responsible for the planning, implementation and/or coordination of all treatment services delivered to clients that are assigned. This includes ensuring that established procedures are followed and that clinical standards that are set for treatment planning, the delivery of individual case management services, group and psychoeducational didactics and discharge planning and referral services are met. Significant creativity and innovation are necessary in order that clients are successfully engaged, motivated and supported in their efforts to achieve treatment plan objectives. Services to be provided using the client centered recovery-oriented model of care. Collaboration and coordination with both internal and external resources are crucial to successful client outcomes.
TASKS AND RESPONSIBILITIES
Service Coordination:
Responsible for service coordination which establishes a framework of action to enable the client to achieve specified goals. It involves collaboration with the client and significant others, coordination of treatment and referral services, liaison activities with community and family and managed care systems, client advocacy, and ongoing evaluation of treatment progress and client needs.
Treatment Planning:
Responsible for the collaborative process in which the counselor and the client develop a written document that identifies important treatment goals, describes measurable, time sensitive action steps toward achieving those goals with expected outcomes and reflects an agreement between counselor and client.
Counseling:
Responsible for the collaborative process that facilitates the client's progress toward mutually determined treatment goals and objectives. Counseling includes methods that are sensitive to individual client characteristics and to the influence of significant others, as well as the client's cultural and social context
Internal and External Communication/Collaboration:
Responsible for facilitating the client's use of available support systems and community and family resources to meet needs identified in clinical evaluation and/or treatment planning. Additionally, responsible for ensuring that the treatment team is aware of the client's treatment goals, recovery process and to receive feedback from the treatment team as well as offering insights and feedback to the team as it relates to all agency participants.
Administration:
Responsible for the accurate, timely and complete documentation of services personally delivered, as well as for ensuring that the documentation of other services accomplished is complete. This position is also responsible for issuing reports and communications that are called for in the completion of assigned duties. Responsible for the efficient operation of the site in conjunction with other team members and ensures that all client activities occur as planned.
Professional Responsibility:
Responsible for adhering to all applicable Federal and State laws, the professional code of ethics prescribed by the agency's license, the professional code of ethics prescribed by the individual's license, the policies and procedures detailed in the Louisville Addiction Center Policy and Procedure Manual, the Louisville Addiction Center Code of Ethics statement and the instruction provided by supervision to include alternative duties. This includes the responsibility to make no statement or take any action that would knowingly threaten the best interests of the agency, other staff members, and/or clients in our care. The holder of this position is also responsible for undertaking and sustaining a commitment to job related professional growth. This position is full time (40 Hours Per Week), following a hybrid work schedule consisting of four in-office days and one remote workday. The work expectations for each setting are as follows:
In-Office Days:
The therapist is expected to be physically present on-site to facilitate group therapy sessions, provide individual counseling, participate in team meetings, and engage in other in-person clinical responsibilities as needed
EDUCATION
: Graduate degree in Social Work/Human Services or related field;
EXPERIENCE
2 years experience in the field of Substance Abuse and/or Mental Health Working knowledge of 12 step programming
QUALIFICATIONS
: Organizational and administrative skills Able to communicate effectively (both written and oral skills) Ability to work independently Ability to multitask Ability to work under pressure Familiarity with health and safety issues
